The MIDAS questionnaire, otherwise known as the 'Migraine
Disability Assessment Questionnaire', is a questionnaire developed to
help physicians and patients assess headaches so that the proper treatment
can be rendered correctly. It measures disability that is caused by headaches
by counting the number of days lost as a result of headaches.
This questionnaire was developed by Professor Richard
Lipton of the Albert Einstein College of Medicine in New York and Dr.
Walter Steward of the Johns Hopkins School of Public Health in Baltimore.

Please answer the all of the following questions regarding
your headaches. After completing the questionnaire, please note the total
at the bottom. This total is used to measure the grade of your headaches,
as determined by your disability level. Please refer to the legend at
the bottom of the table for grade information.
